Be a consumer representative on the Patient Centred Co-Commissioning Group – Cardiology in the Community Sub Committee.
The Cardiology in the Community (CIC) model has been designed with the patient at the centre of the model. It defines an enhanced and optimised approach to the delivery of care, to help support those patients at risk of, or experiencing symptoms of chest pain and heart disease.
WentWest is looking for health consumer representatives to join the Patient Centred Co-Commissioning Group – Cardiology in the Community Sub Committee. The Committee consists of a wide variety of stakeholders including a cardiologist, GP, Pharmacists, nurses, digital health advisors and various others.
The community representative is ideally from Western Sydney and has experience as a patient/consumer/carer in the space surrounding cardiovascular care in either/both the primary or acute setting.
The group meets every six weeks and the next meeting is scheduled for late July. WentWest will pay a sitting fee in line with the WentWest Paid Participation Policy. Consultation with PCCG Organising Group is available via email or phone.
